**UPDATED JUNE 26: Registration for ages 4-4th grade is FULL. Only limited spots remain for 5th-6th graders. For registrants after June 26 there is no guarantee on T-shirt size. Spots could open, so to be placed on a waitlist, send name and age/grade to Anna.

Photo Release

Do you permit the use of photos/videos of your child taken during the hours of VBS to be used for Free City Communications? This includes Free City Facebook/Instagram pages, Free City's website, and email newsletters to parents/church attendees.

Waiver & Liability Release

I, undersigned parent or guardian of the minor listed above, do hereby give permission for my child to attend Vacation Bible School activities sponsored by Free City Church. I waive any and all rights and claims of any nature I may have against Free City Church Church and any organization connected with any of the events, their representatives, successors, and assigns for any and all injuries or damages of any nature which my child as a result of participation in any event. I will assume all risk of any kind that may befall my child as a result of participation in any event. This waiver and release which includes all claims, demands, costs, losses, damages, attorney's fees and liabilities shall be binding on my heirs, administrations and assigns and run in favor of Free City Church. These authorizations shall remain effective through July 21, 2025.

Medical Release

I, undersigned parent or guardian of the minors listed above, do hereby authorize adult workers with the children of Free City Church to consent to any examination, x-ray, anesthetic, medical or surgical diagnosis or treatment and hospital care which is rendered under supervision of any physician or surgeon licensed under the provisions of the Medical Practice Act on the medical staff of a licensed hospital, whether such diagnosis or treatment is rendered at the office of said physician or at said hospital. Further, as parent or guardian of the minor named above, I do hereby expressly consent that my son/daughter may receive emergency medical treatment from any physician, hospital, or other medical center without the necessity of first notifying me, and do further agree to hold blameless any physician, hospital or other medical center rendering such services.

VOLUNTEER FOR VBS

Help kids discover the bigness of God in the smallest of things by volunteering for Free City Church VBS! We are excited to share that this year we are offering a nursery for volunteer’s children aged 0-3. (Please note: Background Check required).
